HIPAA PRIVACY NOTICE

CLAREWOOD HOUSE, INC. conforms with all federal guidelines and regulations under the Health Insurance  Portability and Accountability Act in the protection of both resident and staff health information. 

 

All Clarewood House employees are trained in the protection and privacy of health information. 

 

Clarewood House does electronically transmit/ transfer health care information pertaining to residents and staff, but we do not divulge health care information to anyone or any group without written authorization. We are permitted to use and disclose health care information to treat, bill and collect payment for the services we provide.  We may also disclose your health information for public health activities that are mandated by federal, state, and local governments for collection of information about diseases, vital statistics (births or deaths), or injury by a public health authority. Individuals or their designee have the right to access medical information contained within the facility, including the right to inspect, copy and request corrections to doctor’s notes, X-rays, and laboratory test results.
