The Extended Care Center is a licensed nursing home conveniently located in a one-story, 60-bed building adjacent to Clarewood House tower building and connected to it by an enclosed walkway. Ingeniously designed and furnished for your comfort, this facility offers a single nursing station from which each corridor can be visibly monitored. It has a unique design for privacy in double rooms with TV storage divider, private rooms and several rooms for short-term needs. Each bed has its own window. If you need to spend time there, temporary stays. with special discounts are offered to Clarewood House residents. Those who choose to seek retirement, or assisted living as tower residents, have the extra benefit of being able to age in place. should circumstances change over time. Costs for using the facilities of the Extended Care Center are flexible, based on the extent of care required.

The gracious dining room in the Extended Care Center features a spectacular two-way fireplace facing both the dining room and the living room. Windows in the dining room overlook the grounds of an inside courtyard. There is a physical therapy area, with a whirlpool tub and an adjacent examination room. The pleasant surroundings throughout the facility are color-blended with the outdoor gardens, beautiful artwork and attractive, comfortable furnishings. Guests may visit in the beautiful living room, or in the patient's room, whichever is preferred.

PRIOR TO ADMISSION FROM A HOSPITAL OR OTHER HEALTH CARE FACILITY YOU WILL NEED . . . . .

*  Recent history and physical
*  Progress notes

*  Recent lab results/X-rays/TB test
*  Consultation notes
* Advance Directives (i.e., Living Will, Durable   Power of Attorney).

   If you do not have these items, we will make forms available to you.    
*  Written physician's orders to include medications, diet, 

 treatments, activities and code status.

Please bring: Toiletries, personal articles, marked clothing (usually enough changes of clothing for one week), pictures for hanging, photos and favorite lounge chair (optional) - but please, no other furnishings.
